Kartz Pros ("we") may obtain a consumer report and/or investigative consumer report about you from a consumer reporting agency (Checkr, Inc.) for the purpose of evaluating you for engagement as an installer. This disclosure is provided separately from any other document.

By checking the authorization box, you acknowledge you have received and read this disclosure and you authorize Kartz Pros to obtain such reports now and, to the extent permitted by law, during your engagement. You may request a copy of any report obtained. A summary of your rights under the Fair Credit Reporting Act will be provided.
